Skaters who wish to test or participate in shows or events that are sactioned by USFSA and hosted at other figure skating clubs are required to provide a Permission form that verifies the skater is in good standing and is permitted to participate in the USFSA sanctioned event. Skaters should download and complete the permission form template and send to the Sikumi Figure Skating Club President.
Skaters who plan to test virtually with the Sikumi Figure Skating Club must provide the completed affidavite for each test the skater is submitting. Skaters should provide the affidavit to their coach. All virtual tests require confirmation of the test and that it complies with all test guidelines for approval and submission.
Skaters who are testing virtual should provide this form along with the Affidavit. This form provides consent for the video to be uploaded and shared with the judges for review.
This form can also be used for skaters who wish to allow photos and videos to be used in representation of the Sikumi Figure Skating club on social meida and our figure skating website.
